If I were to troubleshoot this problem. I start with creating new controlfile with different file system and drop the controlfile which got problem. And see it from there.

Was this problem ever resolved?

Every day at 05:00? It's something that is scheduled.

I usually schedule my instance crashes for Friday afternoon, around 4:30 pm.

maybe there is a command: adcron -e to see crashdb sched every 5am
