Back in my designer days I used 'Oracle Designer 2000'; at the time it was just about the only tool available, but as the years & my jobs have since moved on I'm not sure if anything else has come along.

Now if I want to visualize my Db design I just close my eyes & think hard

The best I've used in terms of usability has probably been ER studio, but it's not cheap. I've used Designer but it can be over complicated, depending on what you're trying to achieve.

Currently I'm forced (and I literally mean forced!) to use Visio. In case you hadn't guessed I'm not a big fan of it.

If you're a Java developer go for ARGO/J-DEVELOPER/MAGICDRAW.
ERWin, OracleDesigner, Visio, QDesigner are still available.

But I'd rather use those open source.

Visio? I thot it is used only in drawing flowcharts/process flow
    Not entity relationship diagrams/tables/links.
