You should not try to startup instances on the same machine having the same db_name.

The name of the semaphore, scumntsem_, is created on mounting the database.

When starting another instance, even with a different
ORACLE_SID, the same DBName will cause a conflict with
    the existing semaphore.

Do you have a standby DB on the same machine?

    Jilian,

as I mentioned earlier on my first email, this is a standalone database.

The only thing I could thing off is that we have the shutdown and startup scripts on our backup server to startup and shutdown the database.

I did stop and restart the service and it doesn't help

    Please advise

shutdown the database, stop all services, reboot the box, and start back up.

    Look from metalink:

    Error: OSD 4400
Text: unable to acquire internal semaphore for process
    ---------------------------------------------------------------------------
Cause: Too many ORACLE processes active, maximum number of connections
    exceeded
Action: Delete unused connections, try again

And as I think your platform is NT, if this say nothing to you, the best option with MS when you have not idea of the problem is to shutdown the server and restart

    Regards

    This situation:
ORA-09341: scumnt: unable to mount database
OSD-04400: unable to acquire internal semaphore for process
O/S-Error: (OS 183) Cannot create a file when that file already exists.
may be if any other programm(s) open one or more control files
    or
WinNT stay control file in undefined(locked) stage

Try that: shutdown abort, stop the services, reboot the computer and startup.
