    Error: ORA 1996
    Text: GRANT failed: password file is full
    -------------------------------------------------------------------------------
    Cause: The grant failed when a user could not be added to the password file,
    because all available slots in it were used already.
    Action: Either recreate a larger password file or free up slots in the
    password file by revoking the SYSOPER and SYSDBA privileges from some
    of the remote users.
    You can determine which remote users are valid and their privileges by
    issuing the following query in SQL*DBA:
    SELECT * FROM V$REMOTE_USERS;

    1. Check v$pwfile_users to see the password file users
    2. The issue could be that you have run out of the #password entries, on such case recreate.
    3. If it on NT refer Note:114384.1 from metalink
