    what are your OS and version of Oracle ??
    Do you use Forms Developer version 6.0.5 ???

    its Forms 6.0.5 on NT 4 SP 6 on Oracle 7.3.4

    you're surely facing bug 892342, which has been fixed in Forms 6.0.8.10 ...
    I suggest you upgrade, or look in Metalink for a workaround, but I'm not sure there is one :/
