rootupgrade.sh failed while upgrading oracle restart GI
I am upgrading my standalone grid infrastructure (Oracle restart) from 126.96.36.199 to 188.8.131.52 and ran into issues while running rootupgrade.sh. All the perquisites for upgrade are taken care. Please see the error stack below.

I didn't find any useful information in MOS or in Google for this particular error. Any help would be highly appreciated.
