Standards on Database changes documentation needed

We are looking to put together a best practices for database changes and we're looking for input as to what other dba shops currently do. An example of what is being tossed around is how to identify the machine that the changes are to be implemented on.

The dba's here are being told that the instance name and schema(s) is all we need and the physical machine is not needed since we should already know where the instance resides.

We have over 15 physical machines and I'm sure that other shops have 100's of physical boxes.
