test web form for forms 6i

Hi Friends,

Still beginner in forms 6i...
I am testing the sample form test.fmx provided
by the forms6i installer. This is done by invoking
the web server form tester where the target form
will just display "Forms Server is installed successfully."
But when i submitted or run the web form demo tester,
I got an error "Tha page cannot be found".
The detailed URL desplayed on the ei browser is...

http://test_server/dev60cgi/ifcgi60....lorScheme=teal

Can you please tell what causes the above url to error?
How will the form server know that there is certain database named "test1" with a user scott/tiger?

the "ampersand" is causing it not to
